Enter magnesium — the mineral that’s gained a reputation as the solution to cramping woes. But does the science actually support this reputation, or is this another case of health folklore outrunning the evidence? Let’s have an honest look at what we know, what we don’t, and what might actually help you sleep through the night.

What Actually Happens During a Muscle Cramp?

A muscle cramp is essentially your muscle throwing a tantrum — an involuntary, sustained contraction that refuses to release. Unlike the controlled contractions you use to, say, lift your morning cuppa, a cramp occurs when the muscle contracts without your permission and won’t relax for several seconds to minutes.

The most common culprits are the calf muscles (hence charley horse), the foot, and the hamstrings. When these strike at night, you get the particular joy of being woken from sleep in significant pain — hardly conducive to the recommended 7-9 hours.

Several factors can contribute to cramps: dehydration, electrolyte imbalances, muscle fatigue, prolonged sitting or awkward sleeping positions, certain medications (particularly diuretics and statins), and underlying health conditions. For many people, however, the cause remains frustratingly unclear — these are termed idiopathic cramps, which is medical speak for “we’re not entirely sure why this is happening.”

Magnesium and Muscle Function: The Biological Rationale

Magnesium is an essential mineral involved in over 300 enzymatic reactions in the body. When it comes to muscles specifically, magnesium contributes to normal muscle function — this is an authorised health claim backed by the European Food Safety Authority (EFSA) [2].

Here’s the biological logic: muscle contraction and relaxation depend on a careful balance between calcium and magnesium. Calcium triggers muscle fibres to contract; magnesium helps them relax again. Think of calcium as the accelerator pedal and magnesium as the brakes. The theory goes that insufficient magnesium might impair this relaxation process, potentially contributing to cramping.

Magnesium also contributes to normal functioning of the nervous system and electrolyte balance [2] — both relevant to muscle behaviour. Additionally, low magnesium can actually cause potassium levels to drop, creating a cascade of electrolyte issues [3].

Severe magnesium deficiency (hypomagnesaemia) can indeed cause muscle spasms and cramps as clinical symptoms [3]. The question that keeps researchers busy, however, is whether modest increases in magnesium intake help people who aren’t severely deficient — which is most of us.

What Does the Research Actually Say?

Here’s where we need to be scrupulously honest: the scientific evidence for magnesium supplementation reducing muscle cramps is, to use the technical term, a bit of a mixed bag. Let’s examine the different populations that have been studied.

Older Adults with Nocturnal Leg Cramps

A 2020 Cochrane review — the gold standard of evidence synthesis — analysed multiple randomised controlled trials examining magnesium for nocturnal leg cramps. The conclusion was rather sobering: in unselected populations, particularly older adults, magnesium supplementation showed no significant benefit over placebo [4].

A notable UK-based trial funded by the National Institute for Health Research (NIHR) specifically tested magnesium citrate in older adults experiencing frequent night cramps. The result? Magnesium performed no better than placebo in reducing cramp frequency [5]. This was particularly disappointing given that many people in this demographic have been faithfully taking magnesium for years.

Pregnant Women

Leg cramps are extremely common during pregnancy, particularly in the second and third trimesters. Early small studies suggested magnesium might help, and for a while the evidence seemed more promising for this group. However, a 2021 meta-analysis poured cold water on these hopes, finding no statistically significant difference in cramp frequency between pregnant women taking magnesium and those taking placebo [6].

That said, some midwives and obstetricians still suggest trying magnesium for pregnancy-related cramps, partly because the options are limited (pregnant women can’t take quinine or many other medications) and partly because magnesium is generally considered safe at appropriate doses during pregnancy.

Athletes and Exercise-Associated Cramps

Here’s a surprising gap in the literature: there’s remarkably little rigorous research on magnesium specifically for exercise-associated muscle cramps in athletes. The limited studies that exist haven’t shown clear benefits for preventing those mid-marathon or post-workout cramps [7].

Modern sports science thinking has actually shifted away from the electrolyte imbalance theory of exercise cramps, instead pointing towards neuromuscular fatigue and altered motor neuron excitability. That said, athletes who genuinely are magnesium-deficient — perhaps due to heavy sweating and inadequate dietary intake — may still benefit from correcting that deficiency.

The Deficiency Question

Here’s where things get interesting. Several systematic reviews have noted that if magnesium helps anyone with cramps, it’s most likely to be people who were actually magnesium-deficient to begin with [4][7]. For someone with adequate magnesium status, taking extra may simply result in expensive urine (excess is excreted) or loose stools.

The catch? Dietary surveys consistently show that a significant proportion of UK adults don’t meet the recommended magnesium intake. So while supplementation might not help everyone with cramps, there’s a reasonable argument that it might help those who were unknowingly running low.

Why Magnesium Citrate Specifically?

Not all magnesium supplements are created equal. Walk into any Holland & Barrett or browse any pharmacy website and you’ll encounter a bewildering array of forms: citrate, oxide, glycinate, chloride, malate, L-threonate… It’s enough to give anyone analysis paralysis.

Magnesium citrate has emerged as a popular choice for several practical reasons. Studies comparing different forms have found that citrate is significantly better absorbed than oxide — which, despite containing more elemental magnesium per tablet, passes through the body largely unused (and often causes diarrhoea in the process) [8].

Citrate strikes a reasonable balance between absorption, tolerability, and cost. It was also the form used in several of the clinical trials examining muscle cramps, including the NIHR-funded UK study [5]. Using the same form as the research studies means you’re replicating what was actually tested.

Magnesium glycinate deserves an honourable mention — it’s bound to the amino acid glycine, tends to be very gentle on the digestive system, and absorbs well. It’s an excellent alternative if citrate causes any gastrointestinal upset.

Dosage: Getting the Balance Right

The UK Reference Nutrient Intake (RNI) for magnesium is 300mg daily for men and 270mg for women [9]. This is the amount considered adequate to meet the needs of most healthy adults through diet and any supplements combined.

Clinical trials examining muscle cramps have typically used doses of 300-400mg of elemental magnesium daily, often taken in the evening. This aligns with the RNI and provides a meaningful boost without venturing into excessive territory.

The UK Food Standards Agency advises against exceeding 400mg of magnesium daily from supplements alone, as higher amounts increase the risk of diarrhoea — which rather defeats the purpose of trying to improve your wellbeing [9].

Practical suggestion: Start with 150-200mg daily and see how you tolerate it. If no issues arise after a week or two, you could increase to 300-400mg if desired. Taking magnesium in the evening may help with muscle relaxation overnight, though this isn’t backed by strong evidence — it’s more of a practical consideration.

Timeline: When Might You Notice a Difference?

Magnesium isn’t a quick fix. It doesn’t work like a painkiller where you take it and feel relief within an hour. If it’s going to help at all, most people report noticing changes after 2-4 weeks of consistent daily use. Some studies assessed outcomes at 4-8 weeks.

If you’ve been taking magnesium consistently for 4-6 weeks with absolutely no change in cramp frequency, it’s reasonable to conclude it probably isn’t going to be your solution. At that point, exploring other strategies makes sense.

Safety, Side Effects, and Who Should Be Cautious

Magnesium supplements have a good safety profile for most people at recommended doses. The most common side effect is gastrointestinal disturbance — loose stools or diarrhoea. This is because magnesium has an osmotic effect, drawing water into the intestines. (This is why magnesium oxide is literally sold as a laxative.)

To minimise digestive upset: take magnesium with food, start with a lower dose, consider splitting the dose, or try a form like glycinate which is gentler on the gut.

Important cautions:

  • Kidney disease: People with impaired kidney function should be cautious, as the kidneys may not clear excess magnesium efficiently. Consult your GP before supplementing.
  • Medication interactions: Magnesium can interact with certain antibiotics (like tetracyclines and fluoroquinolones) and thyroid medications. Take them at different times — typically 2-4 hours apart.
  • Heart medications: If you take any heart-related medications, check with your pharmacist or GP.
  • Pregnancy: While magnesium is generally considered safe during pregnancy at normal doses, always consult your midwife or obstetrician first.

The UK Perspective: What Do the Authorities Say?

If you look at the NHS website for advice on leg cramps, you’ll find recommendations for stretching, staying hydrated, and reviewing medications — but you won’t find magnesium championed as a proven treatment. This isn’t an oversight; it reflects the mixed evidence we’ve discussed.

The National Institute for Health and Care Excellence (NICE) acknowledges that magnesium supplements have been used for cramps but notes that results are inconsistent [10]. They don’t recommend for or against — a rather British “we’re not sure, but crack on if you like” stance.

For severe, persistent cramps that significantly affect quality of life, GPs sometimes prescribe quinine sulfate. However, quinine carries notable risks (including rare but serious side effects like blood disorders and cardiac effects), so NICE guidelines suggest it should only be used when cramps are very troublesome and other measures have failed [10].

Given magnesium’s favourable safety profile compared to quinine, many healthcare professionals take a pragmatic view: trying magnesium first is reasonable, low-risk, and might help — even if the evidence isn’t compelling enough for an official endorsement. As one GP might put it: “It’s unlikely to do any harm, and you might be one of the people it helps.”

Beyond Supplements: Other Evidence-Based Strategies

Regardless of whether you try magnesium, these strategies have reasonable evidence behind them:

Stretching

Regular calf stretches, particularly before bed, have been shown to reduce night cramp frequency in several studies [11]. It’s free, has no side effects, and takes about two minutes. Stand facing a wall, step one foot back, keep that heel on the ground, and lean forward until you feel a stretch in the calf. Hold for 30 seconds per side.

Hydration

Dehydration concentrates electrolytes and may contribute to cramping. Drink adequate fluids throughout the day. If you’re exercising heavily or it’s hot weather, consider beverages with electrolytes.

Dietary Magnesium

Before reaching for supplements, consider whether your diet includes magnesium-rich foods: dark leafy greens (spinach, chard), nuts and seeds (almonds, pumpkin seeds, cashews), legumes, whole grains, and yes — dark chocolate. A 30g serving of dark chocolate (70%+ cocoa) provides about 65mg of magnesium. Consider this your scientific justification.

Sleeping Position

Heavy blankets pressing on your feet can keep your toes pointed downward (plantar flexion), which may trigger calf cramps. Try lighter bedding or prop your feet with a pillow to keep them in a neutral position.

When a Cramp Strikes

For acute relief: gently stretch the affected muscle (for calf cramps, pull your toes upward toward your shin), massage the area, and try walking around briefly. The cramp should release within a few minutes.

The Bottom Line

Let’s be honest about what we know and don’t know:

  • Magnesium contributes to normal muscle function — that’s established science.
  • Clinical trials have generally not shown significant benefits of magnesium supplementation for preventing muscle cramps in most populations.
  • People who are genuinely magnesium-deficient are more likely to benefit from supplementation — and many UK adults don’t meet the recommended intake.
  • Magnesium citrate is a well-absorbed, reasonably tolerated form that was used in key clinical trials.
  • The safety profile is good at recommended doses (up to 400mg from supplements), with digestive upset being the main side effect.
  • Stretching, hydration, and dietary approaches should be part of any cramp-prevention strategy.

Is magnesium a miracle cure for muscle cramps? The evidence says no. Could it help some individuals, particularly those with suboptimal magnesium intake? Possibly. Is it a reasonable thing to try given its safety and low cost? Many would say yes.

If you decide to try magnesium, give it a fair trial — at least 4-6 weeks at an appropriate dose. Combine it with stretching and adequate hydration. Keep track of your cramp frequency so you can objectively assess whether it’s helping. And if your cramps are severe, persistent, or accompanied by other symptoms, do see your GP to rule out underlying causes.
